All About Me Family Form – Kindergarten Transition

As your child begins Kindergarten, we invite you to complete our All About Me Family Form, which helps schools better understand each child and support a smooth and welcoming transition.

Or, you are welcome to pick up a printed package at the school office.

Please note: A signed paper Consent to Release of Information form is also required. This allows early care and learning educators to share information to support your child’s transition. The consent form can be returned to the school, your childcare centre, or the School District Board Office.
